Parent Handbook Agreement Form

This confirms that you have read the parent handbook and agree to its policies.

General enrollment form

This is another vehicle for collecting required information about families. 

Topical Permissions form

This allows our staff to apply sunscreen and any other required creams (diapering) to protect your student.

Media Release Form

This gives us permission to use photos we take during the day for future marketing and informational purposes. 

Field Trip Authorization Form

This allows us to take your student on fun activities outside of the facility. 

Asthma Emergency Plan

This form only needs to be filled out if your child has Asthma.

General Health Appraisal

This includes a general health appraisal and immunization records; part of it will need to be completed by the family’s physician. 

Academic Calendar Sign-off

This includes the academic calendar, and sign-off that you have seen our calendar,

Medication Administration

This form only needs to be filled out if there is a medication children must take while at school.

Seizure Emergency Plan

This form only needs to be filled out if your child has seizures.

Allergy Emergency Plan

This form only needs to be filled out if your child has an allergy that requires the use of an Epi-Pen.
